Periodontal Maintenance
Scaling and Root Planing in Manhattan
Tooth loss is not always caused by decay (caries) or injury. In many cases, tooth loss can be directly linked to the presence of gum disease. Gum disease gradually destroys bone, soft tissue, and connective tissue, causing affected teeth to become loose. Treating gum disease early and preventing its recurrence via regular periodontal maintenance is, therefore, one of the best ways to protect your smile.
Manhattan dentist Dr. Matthew Nadler uses advanced techniques like scaling, root-planing, localized antibiotic delivery, and soft tissue laser therapy to help his patients achieve optimal oral health.
What is Gum Disease?
Gum disease occurs when bacteria infiltrate below the gum line, creating a buildup of bacteria called plaque. Once plaque forms below the gums, it becomes very difficult to remove through home cleaning methods. Over time, this plaque hardens and forms tartar; it also causes the gums to become red and inflamed.
Gum disease typically begins as a mild illness called gingivitis. However, if gingivitis is not treated properly, some patients will go on to develop periodontitis. The body’s natural immune response is to fight the bacteria and try to eliminate the toxins created, resulting in inflammation and subsequent bone loss around the teeth. Periodontitis is a severe inflammation of the gums that may cause pus formation, tissue destruction, bone loss, and eventual tooth loss.
What are the Symptoms of Gum Disease?
The early symptoms of gum disease include mild redness, swelling, and having gums that bleed when you brush or floss. Under normal conditions, your gums should never bleed during routine oral care.
As gum disease progresses, you may notice increased swelling, soreness, halitosis (bad breath), and the emission of pus. You may also experience receding gums.

How does Dr. Nadler Treat Gum Disease?
Gingivitis can sometimes be treated with increased home care and routine dental cleanings. For patients with periodontal disease, regular periodontal maintenance is usually necessary. Dr. Nadler uses a combination of scaling, root-planing, localized antibiotics, and soft tissue laser therapy to heal the gums and slow down the progression of periodontal disease.
Scaling and Root Planing
Scaling and root-planing are specialized deep cleaning processes designed to remove plaque and tartar from below the gum line. During scaling and root-planing, the hygienist cleanses any gum “pockets” that have formed due to soft tissue destruction. The hygienist then uses an ultrasonic root cleaner to gently remove rough areas that have developed along the roots of the teeth. This smoothing process creates ideal conditions for tissue regrowth. As healthy new gum tissue forms after treatment, it will closely adhere to each root’s surface, minimizing the chance of bacterial infiltration.
Scaling and root-planing do not typically cause discomfort, and most people report a dramatic reduction in gum soreness and swelling following treatment. Generally, patients have scaling and root planing done every three to four months to slow down the progression of the disease and eliminate inflammation.
While typically the hygienist handles scaling and root planning, in advanced periodontal disease, Dr. Nadler carries out the procedure. Anesthetic is sometimes required in these cases.
Soft Tissue Laser Therapy
Soft tissue laser therapy complements the results of scaling and root planing by encouraging the growth of stronger, healthier gum tissue. Medical lasers can remove bacteria and infected gum tissue more gently than conventional instruments. They also seal off blood vessels and stimulate stem cells, ensuring a quick recovery after deep cleaning.
